Because the fentanyl and Duragesic pain patches are 80 times more powerful than morphine, these manufacturing errors could cause a patients to be exposed to too much fentanyl gel, which could lead to a fatal overdose.

Mylan, along with several other drug companies, manufactures the fentanyl patch. Both fentanyl and its brand name version, Duragesic, have been recalled several times in recent years due to manufacturing defects which could lead to a fatal overdose from direct exposure to fentanyl gel in the patch.

    A videotaped deposition pharmacist Dr. Gerhard Levy in the Duragesic lawsuit filed on behalf of Karin K. Kunnemann against Janssen Pharmaceuticals and Alza Corporation, the manufacturers of the pain patch. The lawsuit alleges that Ms. Kunnemann died as a result of a fentanyl overdose caused by the Duragesic patch.
